Men Lead Demand for New HIV Prevention Injection in Uganda

Uganda is facing an unexpected rush for Lenacapavir, a recently introduced injectable medication for HIV prevention. Health officials have noted a significant uptake, particularly from men, exceeding the initial available stock.

Dr. Herbert Kadama, the Ministry of Health’s coordinator for Pre-Exposure Prophylaxis (PrEP), stated that numerous inquiries are being received daily from various groups seeking the new drug. Lenacapavir, launched in April, is currently allocated to individuals identified as having a very high risk of HIV infection.

The initial rollout saw 19,200 doses distributed to 103 select health facilities. A follow-up consignment is anticipated for October to serve those due for their second injection. However, potential delays are anticipated due to reported shortages of a complementary tablet required as a loading dose by the manufacturer.

Financial constraints remain a significant barrier to broader accessibility. Dr. Peter Mudiope, who oversees HIV prevention at the ministry, indicated that Uganda cannot independently fund large-scale procurement at the current price point, necessitating continued reliance on support from international partners like the Global Fund and the U.S. government.

While clinical trials demonstrating Lenacapavir’s effectiveness as PrEP in Uganda primarily focused on young women in specific districts, these participants are largely guaranteed continued access. The Makerere University-Johns Hopkins University (MUJHU) Research Collaboration is prioritizing pregnant and breastfeeding women, who constitute a substantial portion of the program’s beneficiaries.

Looking ahead, the manufacturer, Gilead, has entered into agreements with several companies to produce generic versions of Lenacapavir. This move is expected to dramatically reduce costs and improve future availability. In the interim, the Ministry of Health is encouraging those at risk of HIV to consider existing PrEP options, including daily oral pills and the every-two-month injectable, Cabotegravir.
