Surgical Errors: When Procedures Lead to Fatal Outcomes in Uganda

Several cases highlight critical lapses in surgical care in Uganda, leading to preventable deaths. Investigations into these incidents reveal recurring issues that demand urgent attention from the medical fraternity.

A tragic incident in March 2008 involved a patient who died post-thyroid surgery due to a post-operative hematoma compressing her airway. The situation was compounded by missing nursing notes and a lack of blood pressure monitoring for several hours, preventing timely intervention.

In October 2010, a 34-year-old woman died during a laparoscopic procedure intended to remove a fibroid. An inquiry found no sound medical reason for the surgery, which was indicated for primary infertility despite the patient not being married or fully investigated for fertility issues. The patient paid Shs8 million for a procedure that ultimately cost her life.

Another case involved a patient operated on for suspected lung cancer based on radiological findings, only to discover during surgery that it was tuberculosis. The patient did not survive the procedure. These instances underscore the importance of thorough pre-operative assessments, including evaluating the necessity of the procedure and ensuring the patient is fit for anesthesia.

Effective patient monitoring, accurate record-keeping, and comprehensive informed consent are crucial elements in preventing surgical-related deaths. Doctors must clearly communicate potential risks and benefits, ensuring patients understand the implications of undergoing surgery. Proper pre-anesthetic evaluations and adequate surgical preparedness, including ensuring blood availability for emergencies, are also vital.
